 \[perl \#119311\] Keep CvDEPTH and savestack in sync

diagnostics
-----------
http://www.cpantesters.org/cpan/report/3db9292c-1474-11e3-9f09-9cca7e66b2dc

This problem can be reduced to :

  $ cat z.t
  use Test::More tests => 1;
  use Scope::Upper qw<unwind SUB UP>;
  sub foo {
  my $r = 123;
  unwind $r, UP SUB;
  }
  sub bar { foo() }
  is bar(), 123;

  $ perl5.19.4_g044aa60-shr-mul -Mblib z.t
  1..1
  not ok 1
  # Failed test at z.t line 8.
  # got: undef
  # expected: '123'
  # Looks like you failed 1 test of 1.

Scope::Upper::unwind used to rely on the stack being preserved when
dounwind() is called. However, starting with change 2537512, dounwind()
can now call leave_scope() which will clean up lexicals in the unwinded
scopes. This causes the loss of $r in the example.

I can fix this on Scope::Upper's side, but I don't know if this change
can have other naughty implications in pure Perl or reasonably sane XS code.

I don’t think it will. In any case, we really need to keep CvDEPTH in
synch with the lexicals, which we could accomplish by moving CvDEPTH
into the savestack, but then the reference count on the sub would also
have to go on the savestack. I think that might have greater repercussions.

I've just uploaded Scope::Upper 0.24 that solves the issue.
